intake assessment

The intake assessment consists of two 1 on 1 meetings up to an hour long each. During your intake we cover everything from posture and movement analysis to injuries, lifestyle habits, goals, and overall health. An intake session is mandatory for us to see you as a client at FMF. All other services are only offered to clients who have had their intake done. If you have questions you’d like to ask prior to booking your intake please send us an email.

Monthly Memberships

We offer 2 types of month to month memberships with 1 on 1 sessions; in person or zoom. With both memberships you are getting custom personalized programming tailored to your needs and goals. This includes access to our online CoachNow program, and equipment starter kit.

in office

Our in office membership consists of weekly sessions, 4 visits per month.

zoom

Our zoom membership consists of weekly sessions on zoom and a full recording of your session. There are 4 visits per month.

6 month Contract: $6,000. *Can be paid in full or in installments.

Professional Athlete Pacage: 6 month Contract: $15,000; $3,000.00 deposit.

Our in office walk in sessions are designed for clients who need a reset after tweaking, pulling, or irritating something and may be experiencing pain. This is a 1 on 1 intensive personalized session including various modalities tailored to your needs including but not limited to targeted soft tissue work, cupping, blading, flossing, trigger point releases and more.

60 Minute One and Done Walk-In Session: $275

Customizeable “Punch Card" Packs:

60 Minutes session: $250.00 per session

30 Minute Session: $125.00 per session

virtual or in Office Form Check

Our virtual walk in session is a 30 minute 1 on 1 session done on zoom. This is designed for doing live feedback form checks for clients on any of our memberships or past clients who may be looking for some feedback. This type of session does not include programming and is only to get personalized attention for acute issues. A recording of the session is provided.

30 minute Session: $125.00 per session
